Yesterday, at the Foundation for Media Alternatives's Privacy and Data Protection workshop, I talked about drafting privacy notices and consent forms. I decided to turn one of my slides into an infographic. Here are guide questions for when you draft your consent form. The answers will serve as your inputs to the form.
REMEMBER: Every organization is different so copying off a template is NOT advisable. Make your own. (I can also help you do that.)
